Application for or renewal of disability benefits from the Maison Départementale pour les Personnes Handicapées (Departmental House for the Disabled)

This form is used to apply for or renew various benefits from the Maison Départementale pour les Personnes Handicapées (MDPH).

 The following requests :

  •  Education allowance for disabled children (AEEH)
  •  Disabled adults' allowance (AAH) and its supplement
  •  Invalidity/ priority/ parking card
  •  Referral to a medico-social establishment or service (ESMS)
  •  Disability compensation benefit (PCH)
  •  Renewal of compensatory allowance (ACTP or ACFP)
  •  Personalised schooling plan - pathways and support for schooling
  •  Vocational guidance and/or training
  •  Recognition of disabled worker status (RQTH)
  •  Free membership of the old-age pension scheme for stay-at-home parents (AVPF)

How do you go about it?

  • Follow the online approach on the Oise MDPH website
  • Send your application by post or drop it off at the Oise MDPH

Documents to be provided :

Required documents: 

  • the MDPH application form
  • the original medical certificate
  • proof of identity of the person making the request
  • proof of address

Deadlines

The MDPH endeavours to respond to as many requests as possible within 4 months. However, some complex applications take longer than 4 months to process. For the PCH, the processing time is longer due to the assessment procedures, which require a visit to the home.
